Circulo Health is a health-tech start-up focused on providing a transparent, connected, and human-centric health model for underserved populations. To support our first physical health clinic, one of the products was a tool that provided the health team with a holistic view of a patient’s health through the Huddle Plan.

User Story Mapping

To understand the ideal journeys of our users, I started by doing a User Story Mapping exercise with our health location team members. We wanted to create a revolutionary health model that focused on comprehensive health and what it took to keep people healthy, instead of waiting for them to get sick.

The product and technology teams came together to identify touch points for the members, health team, as well as the back-end technology required to support each of the scenes.

This service map allowed the team to visualize the experience and pinpoint various tracks of work.

CHALLENGE

The existing Electronic Medical Record (EMR) system was difficult to integrate into its users’ workflow, and it was challenging to input patient information or obtain a holistic view of a patient’s health.

SOLUTION

The Huddle Plan web application aimed to integrate vital information so that teams could easily “huddle” on a patient’s health plan. We brought in patient motivators, attached action items to patient goals, and visitation notes, with data integrations that allowed the health team to work within a single user-friendly web app, instead of toggling multiple tools.

RESULTS

We grew our patient waitlist from 0 to 400 in 8 weeks and gained 60 new members within the first two months of operation, most of whom came in multiple times to utilize our services.

Information flow

Visualizing what APIs exist and how information would flow from the EMR system to our Huddle Plan tool for the v1 launch.

Feature prioritization

Working with various stakeholders to vet features. User needs, business needs, and dev efforts were all taken into account to come up with a list of v1 features that would be built out. It was great to have everyone on the same page.
